Efrain Hernandez was tragically killed when the van that he was a passenger in collided with a pickup truck in foggy conditions. Another passenger in the van also died in the accident which occurred as the van was eastbound on Floral Avenue and was attempting to cross Highway 41. According to the California Highway Patrol the driver of the van was unable to see a pickup truck traveling northbound in the fast lane and the vehicles collided.
